The following is the waiver policy for your child’s participation. Kindly review it in full before proceeding.
I understand that participation in the Future Star Program with Progressive Soccer Ozzy involves physical activity and carries a risk of injury. I voluntarily accept and assume all risks associated with participation.
I authorize Progressive Soccer Ozzy staff to seek and obtain medical care in the event of an emergency. I understand that every effort will be made to contact me before any treatment is administered.
I hereby release and hold harmless Progressive Soccer Ozzy, its staff, coaches, volunteers, Azusa Pacific University, and any affiliated organizations from any liability, claims, or demands arising from my child’s participation in the program.
I grant permission to Progressive Soccer Ozzy to photograph and/or video record my child during program activities. I understand that these photos or videos may be used for promotional and marketing purposes, including but not limited to social media, flyers, and website content.
No personal information or names will be shared without additional consent.
